Medication errors are widely acknowledged and well-known as a significant problem causing serious adverse effects that may lead to death. There are many interrelated factors that lead to medication errors. Regardless of how they are made, medication errors can increase patients’ length of stay, leading to an increase in the cost of healthcare. For the safety of patients and the wholeness of healthcare systems, it is crucial that healthcare professionals work together to carefully assess, detect, and understand the nature and sources of medication errors. The purpose of this article is to assess healthcare professional and system failures that lead to medication errors and to suggest guidelines and prevention strategies to reduce their incidence.
